Informed Consent to Telehealth

This Informed Consent describes the nature of telehealth services delivered through the Taggs platform ("Taggs") and asks you to acknowledge the benefits, risks, and limitations before you receive care.

What telehealth is

Telehealth is the delivery of healthcare services using secure electronic communications (such as messaging, questionnaires, photos, phone, or video) when the patient and clinician are in different locations. On Taggs, most visits are asynchronous — you complete an intake and a licensed clinician reviews it and communicates with you through the platform.

Potential benefits

Convenient access to a US-licensed clinician; faster follow-up and refills; the ability to be evaluated and treated without traveling to an in-person office.

Potential risks & limitations

Telehealth cannot fully replace a hands-on physical examination. Information you provide may be incomplete or inaccurate, which can affect clinical decisions. Technical issues (connectivity, device failures, security events) may delay or disrupt care. In rare cases, your clinician may determine that telehealth is not appropriate and recommend an in-person visit.

No guarantee of outcome

No specific outcome is guaranteed. Your clinician may decline to prescribe a medication if it is not clinically appropriate for you.

Emergencies

Taggs is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.

Privacy & records

Communications and records created through Taggs are stored and protected as described in our Health Data Privacy Notice and Privacy Policy. Standard electronic communications carry some risk of interception; Taggs uses reasonable safeguards to reduce that risk.

Your rights

You may withhold or withdraw consent to telehealth at any time without affecting your right to future care. You may request a copy of your medical record. You may ask questions about your diagnosis, treatment options, alternatives, and expected outcomes at any time.

Independent clinicians

Clinicians providing care through Taggs are independent professionals licensed in your state. They are solely responsible for clinical decisions and are not employees of Taggs.

Acknowledgment

By completing an intake or continuing to receive services through Taggs, you acknowledge that you have read, understood, and consent to receive care via telehealth on the terms above.
